Referral Process
When making a referral, it is greatly appreciated if you are able to provide BCC with the information below.
Please call our Admissions Clinical Care Coordinator at
610.373.4281, Ext: 2726, OR email lmessner@berkscc.org.
- Patient level of care (MH; D&A; DUAL; IOP; or IOP DUAL)
- Demographic information
- Insurance information (please note that should the patient have private insurance, it may take up to two days to verify prior to scheduling an appointment)
If patient is currently receiving inpatient services, please also provide discharge paperwork
OR most recent psych evaluation
by faxing to our confidential number at 610.373.3779.
